Rather than labelling people by their disease, physicians, clinicians, scientists and all partners within the CarePath consortium recognise the need to understand and address the unique barriers to dialogue associated with obesity, type 2 diabetes and cardiovascular disease as well as all other chronic health condition.<\/p>\n
