CarePath wordt in de praktijk getest in zes Europese landen: Duitsland, Israël, Nederland, Polen, Spanje en Zweden. Deze pilotsites omvatten zowel eerstelijns- als tweedelijnsgezondheidszorgomgevingen, zodat de CarePath toolbox werkt voor mensen in verschillende gezondheidszorgsystemen en gemeenschappen.

Door het uitvoeren van drie pilotstudies in verschillende regio's en een proof-of-conceptstudie in Duitsland verzamelen we waardevolle inzichten om praktische, schaalbare oplossingen te creëren die voldoen aan de behoeften van patiënten, families en zorgteams overal ter wereld.
