CarePath is being tested in real-world settings across six European countries: Germany, Israel, the Netherlands, Poland, Spain, and Sweden. These pilot sites include both primary and secondary care settings, ensuring that the CarePath toolbox works for people in diverse healthcare systems and communities.

By running three pilot studies in different regions and a proof-of-concept study in Germany, we are gathering valuable insights to create practical, scalable solutions that meet the needs of patients, families, and care teams everywhere.
